Participating in the many daily programs, events and activities will improve the resident's self-esteem and increase self-confidence. Our dedicated activities staff looks forward to enhancing every day with new experiences. Our social service team assists both residents and their families during transitional periods. We also provide essential information, manage requests and concerns, and help in care planning and discharge planning for each resident. We can arrange professional services for:

Creating an enjoyable dining experience for our residents is a priority. The menus for Premier are planned by a Registered Dietitian to promote proper nutrition and overall appeal through food preparation and presentation. Special diets can be tailored and are readily available to meet your unique nutritional needs.

If you prefer to dine in your own room, we will be happy to provide you with in-room dining.

The cost of care will vary based on several factors. Some of the costs may be covered by insurance, Medicare or MediCaid. We have provided some information about these coverages below. Insurance
Premier Health and Rehabilitation accepts many health insurance plans. While the benefits and out of pocket cost will vary, many plans will cover at least a portion of the care provided for a skilled stay. Insurance often falls into three categories:

  • Fee-for-service health insurance plans
  • Health Maintenance Organizations (HMOs)
  • Preferred Provider Organizations (PPOs)


Medicare
Medicare replacement insurance or a Medicare Advantage Plan is a type of Medicare health plan offered by a private company that contracts with Medicare. While we contract with many of these plans, please call the facility directly to confirm they are a contracted provider.

Managed care refers to a method of providing health care to people who have purchased health insurance from companies that use this method with the goals of reducing the cost of providing health benefits and improving the quality of care. Most managed care plans require that enrollees only seek health care from their approved providers that have met their standards for outcome and are within their contracted network.

Medicaid
If you meet eligibility requirements, Medicaid may help pay for nursing home care. Each state determines its own standards and requirements. In most cases, qualified residents are required to make a co-payment that consists of their monthly income (Social Security check, pension, etc.), less a personal allowance. However, the amount you are required to pay is determined by your local Medicaid office.

There are times when caregivers need a helping hand. It may be a need to attend a special event or just to get a well-deserved break. Nursing Facility offers respite services to provide care when the caregiver cannot. Our team is experienced in caring for individuals with a wide variety of service needs, including a decline in memory. We offer respite care that can provide peace of mind for the caregiver and a positive, social experience for the person served.

Services provided include:
  • 24 hour nursing
  • Medication administration
  • Dietician approved meals
  • Assistance with personal activities of daily living
  • Activities and socialization opportunities

Q: How long will I have to stay?

A: An average stay is between 2-3 weeks.

Q: How is my laundry handled?

A: Laundry can be handled in two ways. Your family may take it home or we can provide laundry service for you.

Q: What is a typical day like?

A: Breakfast is served around 8 am. If needed, you will receive assistance with getting dressed. Therapy is scheduled at a specific time. Lunch is served at 12 noon. Therapy may again be scheduled in the afternoon. We start serving dinner at 5pm. Medication is provided to you throughout the day.

Q: How will I pay for my stay?

A: Most insurances will cover all or a part of your stay. Please speak with our Business Office staff to discuss specific insurances.

Q: What should I bring?

A: Everything you needed for the hospital. Comfortable clothing are needed during therapy time as well as appropriate shoes.

Q: What about TV’s and phones?

A: They are both available.

Q: Can I get my hair styled?

A: Yes. We have hairdressers available 2 days a week.

Q: What are the visiting hours?

A: You may have visitors at any time, but we recommend between the hours of 8:00am and 8:00pm.

Q: Are my guests allowed to dine with me?

A: Yes. Guest meals are available at a cost.
